Standardized on one LICENSE file, added LICENSE where it was missing,… (#4912)

* Standardized on one LICENSE file, added LICENSE where it was missing, add NOTICE file wherever there was a LICENSE file.

* Got rid of all of the subdirectory LICENSE files.  In NOTES, I added copyright assertions for all of the included code that I could find.

* Removed all of the extraneous sub-dir NOTICE files that I'd added earlier in this branch..

* Added copyright assertions for all of the included code that I could find.
