Create nodes from 12 word (BIP-39) phrase by default

hsm_secret will now be 12 words instead of 32 bytes.

1. Update hsmtool to handle this.
2. Update code to handle this new format.
3. Generate this new format by default.
4. Add hsmtool to convert (if they generated the original from 12 words!)
