Stop auto-filling Name and Description

We're currently using an auto-filled Name and Description on Final Review. They are usually pretty crap, but the presence of a Name and Description lulls the approver. It's not super onorous for Final Review to make a Name, and Description is not really needed.

• name should not be auto-generated by any mechanism; we can revisit later
• description should not be marked as required (I have been successfully submitting them blank)
• description should not be auto-filled—we get significant junk, and descriptions are only really needed in some cases.
