I'm assuming it is a bug... will investigate further.
If it isn't a bug, I think maybe it would be useful to improve the message.

Ah, it happens when the same qubit is used more than once for a gate.

Perhaps the message could be improved to say something more like:

Gates cannot operate on the same qubit more than once

As well as be an error and not a panic.
