Badge Artwork Credits get duplicated

Describe the bug
When adding badge artwork credit to an achievement, a duplicate credit will be created. This duplicate will copy the user and timestamp from the preceding logic credit. There doesn't appear to be a way to delete the credit.

To Reproduce
Steps to reproduce the behavior:

1. Go to an achievement's management page
2. Click "Add contribution credit"
3. Enter the task and user
4. Click "Create"

Expected behavior
A single credit will be created with the given user and timestamp.

I believe we intentionally designed it this way. I agree it seems unexpected and cumbersome.
