Add time to read the instructions of a form?

Could have sworn that we had added this, but I can only find that time_to_answer uses how long it takes to write a response.

"The effect of word predictability on reading time is logarithmic" might be useful here, if we want more than basic word-per-minute reading estimates.

It used to be in ratemyPDF (just a straight reading speed multiple of the length of the words) but I removed it. I thought it was a bit cluttered/confusing how it related to time to answer and redundant with the word count.
