- extends a description of methodology, which includes usage of ADDA through the databases in addition to the direct one - marks 7 papers as the database ones - adds 56 new papers (mostly indirect usage) for years 2011-2016 - changes the style to show no more than 9 coauthors - adds Chile, Taiwan to the map (Taiwan is not counted as a separate country).

Removed redundant indentation, a few minor formatting changes (added some blank lines). Added TOC placeholders. diff works awfully on these changes
Removed `` for ADDA and other code names, since it looks ugly at GitHub. But they are retained for file names, citations from the code, etc. Also removed extra "" around ``, since `` is already sufficient for visual separation.
