This File copies in forms.css and a formfoolery.js file to work in conjunction with the plugin. Please include them in your application layout.

Be sure and read the README file. It's located at vendor/plugins/labeled_form_builder/README or just run rake rdoc in the plugin directory for some documentation on the whole thing
