An incident was opened on 24 October that wasn't noticed until 10 February. Presumably the usual alerts were emailed to [email protected], although there's no record of them https://groups.google.com/a/digital.cabinet-office.gov.uk/g/govgraph-developers. Emails would only have been sent for a few days. The fact that the incident was still open prevented new incidents being opened.

Maybe emails about incidents are being missed/forgotten/ignored. Slack notifications might be more noticeable.
