chore: refactor middlewares so that if a middleware isn't called for, it isn't installed, rather than installing a no-op variant

This can be seen with the ServerReporter middleware. If it's not called for (i.e. not configured), we still install it, but with no-op internals. It seems like it'd be better if we weren't installing them at all.
