Add "display text" attribute to data elements in schema

For applications, it is sometimes desirable to show the data element in title or sentence case. Often, the description provides this, but it also can be much more verbose. Alternatively, maybe it's possible to simply convert the snake_case to Title Case without introducing repetition.

Proposal: Rely on case conversion, but allow it to be overridden with "display text".
