What is the problem? (only list ONE problem per report)
When i start the pairing the adapter lights stops and controller blinks then after the controller stops blinking the adapter light slowly blinks again, so i think the pairing failed
What did you expect to happen?
For it to pair since this controller has a ds4 mode and switch mode

hardwaretester shows it as Sony PS4 Dualshock 054c-09cc-Wireless Controller
