Hangs on Form Submit

I have a few website's that hang on Form Submit of a new comment:

I've tried to look over again at the Read Me and seems all three steps are in place to have the plugin working, so not sure what's missing?

Using node.js 0.10.7 locally and node >=0.8 on the servers.
