feature: Relate the validation error back to the record and field causing it

Describe the Feature Request
Please provide more context in the error log. Many errors appear there multiple times (for obvious reason). I suggest adding the cell from where the error originates.

We are aware of this issue, this requires a coordination with the FHIR server because we do the operations in "batch". Hence, we most of the time do not know which record inside the batch caused the problem. Let's make this an issue and work on that.
