[Bug]: UCI Health is Returning a 500 Error Response #586

What happened?
When selecting a source from UC San Diego (My provider is UCI Health Tustin), I select the provider option which is in the source database with an ID of 3e17c327-e633-415a-a4a4-8a659b75661e. This takes me to the "lighthouse.fastenhealth.com" redirect link, but says that the page is currently unable to handle this request with an HTTP Error 500.
