Create group form not cleared after submitting

Describe the bug
After creating a group, if you attempt to create a new group, the name field isn't cleared and is filled in with the name of the previously created group.

To Reproduce
Steps to reproduce the behavior:

1. Create a group with the name test
2. Create another group, note the name field is already populated with `test

Expected behavior
The name field should be cleared as part of creating the group/hiding the modal.
