Create an issue form from our blank issue template

Overview

To make Issues less intimidating, I'd like to create issue forms to get specific information out of the submitter. We're going to start with the Blank Issue form.

Action Items

• using the Blank Issue template in the .github folder, create an issue form called Blank Issue Form (very creative, I know)

Issue Form Documentation - from a quick glance you may have to convert from MD to YAML

• translate the current issue template as is, we'll add to it later as needed
