Site-specific path text

The same text is in path reports regardless of site. Suzanne and I will generate some standard text for certain sites and histologies. We'll start with a small number of sites that we can use for demos. The other sites can get the generic text or maybe we should make it even more generic because it is really strange to see text about the left breast in an HL7 with epath site of C619.

Once we have the text, this should be pretty straightforward.

We created some fake HL7 with better text than we is currently used in the generator. We should use that and improve the generated text.

We need to be sure that female is not assigned to a prostate. Otherwise, I don't see any mention of sex in the text. And the age of the patient should probably be 55+.
