Not Replacing All <img Tags? #7

According to a review by deluigiluca, the plugin only added the attribute to the first 9 of 40 images. Here's what this person said:
Yes, it replaces <img with <img loading="lazy" But... - it replaced only 9 <img on 41 on my page ... anyway, thanks for trying to do the web a better place!
Let's test this and fix it if we can replicate the issue.
