[Bug]: when a document fails to ingest successfully, it disappears from knowledge #851

Bug Description
When ingesting documents and they fail, they appear in the knowledge section as failed and you can inspect the error that caused the failure.
However, if you refresh, or otherwise leave and return to, the page those documents and the reasons for failure are gone making it hard to see which documents you need to re-ingest or what went wrong so that you can try to fix it.
Steps to Reproduce
- Go to Knowledge
- Choose to ingest a whole folder
- Ingest a reasonable number of files that will cause a failure (for me, ingesting a reasonable number of files results in failures)
- Refresh the knowledge page once the failures have happened
Expected Behavior
I should see the failed documents and the reasons why they failed.
Actual Behavior
The documents disappear leaving only the successfully ingested documents.
