Create example instrument with monitor that has an efficiency

Recipe:

1. Make an Arm() with an extend block
2. Extend measures wavelength and applies intensity*efficiency(\lambda) in instr bar
3. Uses Monitor_nD to measure that variable
4. Optionally “eats” 1-eff of the beam
