Should the included-package list be keyed by both package and version? #134

My instinct is that it should probably only be keyed by the package name, and that the version is a mandatory parameter. Specifically, I'm not sure that a package would ever want to include more than one version of a given package.

included-package list should only be keyed by package name. Will fix in the next version.
