Add a "Bolus Mode" configurable pill to Nightscout

Loop and AAPS pump software can operate in a manual mode.

Nightscout shows Loop bolus amounts and other data but does not indicate whether Loop is operating in Manual Bolus or Automatic Bolus mode. Knowing the bolus mode provides insight into why Loop may limit bolus amounts (e.g., 40% max auto bolus) or whether caregiver manual boluses are necessary.

It would be helpful for Nightscout to display a "BOLUS MODE" in the top status bar to give caregivers and clinicians immediate remote visibility into whether Loop is delivering insulin via automatic bolus or manual bolus, helping explain insulin dosing behavior and improve management decisions.

This will require updates to Loop and AAPS status upload payload for bolus mode (auto or manual).
