TOTP is a shared-secret system. But nostr has no central account management. Also, if a private key is leaked, an imposter can post as the user from anywhere because nostr has no central gatekeeping. Maybe I misunderstand the context you are thinking about, but it doesn't work with nostr accounts in general.
