At this time, the ordering of fields in the contribution form is:

• Rec type
• Form type
• Contributor type
• Name title, first last suffix
• Address, Address2, City, State, Zip
• Is out of state pac, Out of state pac
• Date
• Amount
• In kind description
• Employer, Occupation

Suggest something like:

• Rec type -> rename "Contribution type"
• Form type-> remove (calculate automatically)
• Contributor type
• Date
• Amount
• In kind description
• Name title, first last suffix
• Address, Address2, City, State, Zip
• Is out of state pac, Out of state pac -> "Is out of state PAC?", "Out
  of state PAC id"
• Employer, Occupation
