How to diagnose why 'something' is not cached?

My database logging system show me that this query is executed 33 times/sec

SELECT `option_value` FROM `wp_options` WHERE `option_name` = ? LIMIT ? |  

Now: is there a way to diagnose why the plugin is not helping reducing numbers of query like this?
