Revamp issue UX

A few points:

• Add a consent and licensing questionnaire
• Ask for their consent to be tagged on social media and for their pictures to be taken (the latter is important as we sometimes have speakers under 18)
• Ask if they are open to sharing their pictures for posters or if we can grab one from their social media handles
• Add "Agreements" and links to the Code of Conduct
