missing episodes

Describe the bug

This has been happening for years, and I have over time submitted bug reports on this issue.
What happens is I see a download I've been waiting for in the download folder.
I expect Medusa to process the download and move it to it's finial show location.
NOTE: this works correctly most of the time..
Latter I will go to watch that episode and it's not available.
I'll search for it, no where to be found, not in recycle bin or any where on the disks.
The logs show it being processed and should be where it's not.
SO what happen to it, the mystery
On downloads I'm watching for I manually move them, If I see them in the download folder as I don't want to take a chance.

Question to the developers,

Is there a situation were an episode is to be moved to it's final destination and can't be for some unknown reason like a drive is not available or busy etc... What does Medusa do with that file?
I ask because I have noticed that when an episode is to replace an existing episode, the old episode is removed and sometimes the new episode vanishes.

NONE.. I cant find a reason why it's happening other than Medusa is removing the episode or not moving it.

Every episode should make it to it's final destination, if not an error should be produced.

no logs submitted since they show no error,
actually they show that the episode was moved when in fact it has not.
