SAMPLE SQL document

There was an extra document called Cookbook-SQL_Examples_9.7 and it was referred in the book as: "The following documents are also available for free from my web site: SAMPLE SQL"

I have that document, but it could be converted into files to be more practical to execute them directly in the database. For example, this set of sample files could be published as a release from the GitHub project.

In other words, the appendix could be created into a file, and convert the data into a CSV file or INSERTs.
