dg.render() won't render a form, unless the form is wrapped in an element

  dg.modal({
    _theme: 'form',
    _id: 'SearchControlsForm'
  });

Works

  var element = {};
  element.controls = {
    _theme: 'form',
    _id: 'SearchControlsForm'
  };
  dg.modal(element);

This could have to do with the Form being loaded in a modal though...
