bug: Failed to open session

Can't reliably reproduce but every so often when I start a fresh session the initial message will go through, llm will start responding but I will see the toast in top right corner Failed to open session.

Will add logs (and share link) if I can get it again. This has happened a handful of times but I just forgot to track it.
