References are duplicated

Heya,

When I make any changes to a character file in a new project, it adds:

```
* Appearances in Chapters
Not yet referenced in story.
```

If I edit, and save agaain, it adds the section over and over.

I'm getting the same with the Glossary section when editing a chapter.

I'm on Emacs 30.1
