Install gRPC PHP extension (disabled, by default)

Thanks for the wonderful product.

I've contributed to a library which relies on ext-grpc for PHP (`pecl install grpc`), and the extension takes ~3 min to install for every job in the build. Is it possible to have grpc pre-installed so that I just need to enable it via ini?
