How do u create and load extensions?

is this outdated? - i mean README.md

Not really sure if this is right place to ask this, i was trying to make a extension and it is build in target but everytime i try to load it - it says file not present.

I did tried to refer to an existing pr (for eg:csv) but i think things have changed since then.
