For some reason I don't get any syntax highlighting on the file. Adding tasks from the left panel doesn't work. It seems like todo.md is seen as a markdown file.

There is no steps to reproduce and I don't own a Mac.

Any errors in devtools workbench.action.toggleDevTools Developer: Toggle Developer Tools?

No, no errors. It seems like it's not doing anything.

Also then I click on open default file (from the activity on the right), I get a message that Default file does not exist. Then when I specify, nothing happens.

Same here. It works for me after renaming the file to todo.txt and changing the user setting to include its pattern, e.g. "todomd.activatePattern": "**/todo.{md,txt}"

Maybe extension wasn't activated at all?

When this happens - does it show activation time like 11ms or is it showing "Activating..." forever or something like that?
