telescope picker for providers

I created this simple telescope picker for providers, to make the :AvanteSwitchProvider x simpler. Well, claude made it for me :)

I don't know how to create a real telescope plugin from this or if it is even needed, but maybe someone can benefit from this post or maybe this can be added to the tricks wiki page.
