Updated Readme for new file locaitons and made installation easier

Hey there,
i added support for the newer zabbix agent 2 by creating a new sh file.
The template for the zabbix server does not need any changes.
I tested it with:
I also updated the Readme to the new file locations and made minor changes to make the installation easier.
