This PR makes two main changes:
ContextSamplernow only samples documents from an iterator - it returns docs, nothing more.build_qa_turn()handles all prompt formatting - a single method that takes a question/choice/answer and returns the formatted messages, making it obvious where delimiters, prefixes, and chat roles are applied
