Database editor tracking

I want to be know who and when made changes to the database
In order to be sure that the data is correct

Questions

• If we don't use Github to store the data, how do we achieve that?
  • Do we need something more than user + date + comment/reasoning?
• Can we assume it's only admins updating the data?

If it's Github, do we actually need to store that in the database?
