Issue reporting guidelines

In order to help contributors and users better tackle and filter through reported issues, it would be beneficial to introduce a title template. this will also help users who have issues better scan through them to find a fellow who reported a similar problem.

I suggest using this prefix:

[STino_VERSION][SublimeText_VERSION]

**examples:

• [STino][ST2] issue uploading with programmer => first version of STino on Sublime Text 2

• [new-STino][ST3] missing snippets for... => second version of STino (branch new-stino) on Sublime Text 3
