Reorder operation fields & refactor path item

Problem

1. Operation fields are ordered very differently from one file to the next. This makes it confusing to work across multiple files.
2. Most specs refactor the path item when it is not needed.

Solution

1. Reorder operation fields according to order defined in the OpenAPI v3.1 spec.
2. Remove path item from components where applicable.
