Support manual control of the bulk operation #42

Allows a consumer to control the bulk operation explicitly. This is useful when we only want to store documents if a document with the same ID does not already exist (using the create mode) in the index. The current behaviour defaults to index when an ID is present, which may result in a new version being stored in such circumstances.
