Add SafeSearch and Region configuration support

Add DDG_SAFE_SEARCH env var (STRICT/MODERATE/OFF)
Add DDG_REGION env var for default region
Add optional region parameter to search tool
Update README with configuration examples
Maintain backward compatibility

Features:

SafeSearch uses DuckDuckGo official kp parameter
Region uses DuckDuckGo official kl parameter
AI can override region per query
Fully tested with multiple regions
