The `request`, `csrf_token` and `model_opts` variables are no longer available as of https://github.com/wagtail/wagtail/commit/146a9f8e3f02e56057410bb3fe2c784b61b0c47e/ , due to the button appearing within the slim_header.html include which is included using the `only` option.
